Find a Doctor or Hospital
Using in-network providers is an easy way to save money on your out-of-pocket costs and receive the greatest value from your health insurance plan. CoOportunity Health plans are "open access" plans. This means you can schedule an appointment with any provider without a referral from a primary care provider.
The directories are organized by product (see gray toolbar above to select your product). After choosing your product, you can search for a provider based on location and specialty.
Although we update our directories on an ongoing basis, it is a good habit to ask if a provider is in-network before receiving care.
Provider Qualifications
Our directories include provider address, phone number and specialty. If you would like more information about a provider's professional qualifications, including where they completed medical school and residency training, please call Midlands Choice Customer Services at 1.800.605.8259, option 5.
For all other questions related to your health plan, benefit information or general provider information, please call CoOportunity Health Member Services at 1.888.324.2064. Hours are Monday through Friday, 7 a.m. to 6 p.m. CT (TTY at 1.888.850.4762).
Mental Health/Chemical Health Providers
If you need help finding or selecting a mental health or chemical health professional in your network, please call Member Services and ask to speak with one of the professionals staffing the Behavioral Health Personalized Assistance Line (PAL).
Hospital Services
Non-emergency hospital services are coordinated by your treating physician. In an emergency, dial 9-1-1 or go to the nearest emergency facility.
