As an entity that receives federal loan funding from the Centers for Medicare & Medicaid (CMS), we are required to make public important company documents that describe our operating structure. CoOportunity Health also believes in transparency of information to its members, regulators and the public.

Company Bylaws

CoOportunity Health Bylaws outline the governance structure of the company as an Iowa nonprofit corporation organized under the Revised Iowa Nonprofit Corporation Act, Chapter 504, section 504.206, Code of Iowa.

Articles of Incorporation

CoOportunity Health filed Articles of Incorporation with the Iowa Secretary of State on July 20, 2012 and amended on March 3,2014.

Certificates of Authority

CoOportunity Health is a licensed health insurance company in the states of Iowa and Nebraska. The Certificate of Authority to provide health insurance in Iowa was issued by the Iowa Insurance Division on March 22, 2013. The Certificate of Authority to provide health insurance in Nebraska was issued by the Nebraska Department of Insurance on May 1, 2013.
