Free office visits? That’s right. Each family member who has benefits gets three free office visits. This includes primary care, specialists, chiropractors and outpatient behavioral health. For many people, their free preventive care and three free office visits will cover their healthcare needs for the year.

How Does Three-for-Free Work?

CoOportunity Health wants to make sure you receive the care you need. We understand that cost is a consideration for many people when deciding whether to seek care. To make it more affordable, we include Three-for-Free feature with many of our plans.

• CoOportunity Health pays the doctor’s fees for the first three office visits for each person who has coverage in a plan year.

• Office visits included are primary care, specialists, outpatient behavioral health, urgent care, and convenient care/walk-in clinics. Applies to in-network or Tier 1 providers only.

• If you need services such as X-ray or a lab test, you’ll be responsible for those costs based on the cost-sharing of your benefit plan.

• This feature is included in all Silver, Gold and Platinum level plans for non-HSA products.

• After you receive three free visits, your out-of-pocket costs will depend on your deductible, copay and coinsurance. Once your reach your out-of-pocket limit, you won’t have any more out-of-pocket costs for covered services.
