Your Personal Information
“Personal Information” refers to specific data elements that can be used to identify or contact you as an individual. Personal Information includes, for example, your name, home address, phone number, birth date, gender, email address or other information that is unique to you. CoOportunity Health collects Personal Information on the Site only when you supply it, such as when you send an e-mail or sign up for or purchase a program, product or service through the Site, or when you visit the Site.
CoOportunity Health collects information about visitors to the Site from “cookies” and the Internet Protocol (“IP”) addresses of the computers used by visitors to access the Site.
